What Is a Biostimulator? Meet Sculptra

By Joanna Ferris, RN · August 13, 2026 · 6 min read

What if rejuvenation wasn't about adding more — but encouraging your skin to do more?

That is one of the reasons I love talking about biostimulators.

In medical aesthetics, we often hear about treatments that soften a line, replace volume or create an immediate visible change. Biostimulators approach rejuvenation a little differently. Instead of focusing only on what we can add to the face today, they work by stimulating the body's own processes — particularly collagen production — so changes develop gradually over time.

So, what exactly is Sculptra?

Sculptra is an injectable biostimulator made with poly-L-lactic acid (PLLA). Once introduced into appropriate areas of the face, it helps stimulate your own collagen production and support the underlying structure of the skin.

That means Sculptra isn't really about walking into the clinic looking one way and walking out looking dramatically different. It's more of a slow-build treatment.

As collagen production is stimulated, we may begin to see gradual improvements in things like facial support, skin quality, firmness and the appearance of volume loss. Results vary from person to person and develop over time.

And personally, I think there is something really beautiful about that approach.

Sculptra vs. traditional filler

This is one of the biggest questions I get.

Traditional hyaluronic-acid dermal fillers are often chosen when we want to strategically replace or add volume and can provide an immediate visible change.

Sculptra has a different job. While the injection itself initially contains fluid used to deliver the product, the goal of Sculptra is to stimulate collagen and gradually improve the skin's structural support.

Neither approach is inherently 'better.' They are simply different tools — and sometimes they can even play different roles within the same long-term rejuvenation plan.

Why are we talking so much about collagen?

Collagen is one of the important structural proteins that helps give our skin support.

As we age, changes in collagen are one of the factors that contribute to the visible signs we associate with aging. Instead of trying to chase every individual line as it appears, treatments such as Sculptra give us another way to think about rejuvenation: supporting the foundation.

That is very much how I like to approach aesthetics at The Ferris Me. Not changing your face. Not trying to make you look 20 forever. Not chasing every trend that comes across social media. Just looking at your face, your skin, how it is changing, and asking: what could we do to help you continue to look like you?

Is Sculptra a one-and-done treatment?

Usually, we approach Sculptra as a treatment series rather than a single appointment. Galderma describes a typical treatment course as approximately three sessions over a few months, although the appropriate number and timing of treatments need to be individualized.

And because we are asking your body to produce collagen, patience is part of the process. This is not necessarily the treatment for someone looking for a dramatic change by next weekend. It may be a beautiful option for someone thinking more about how they want to age over the next several years.

The Ferris Me approach

Biostimulators are one more tool in our increasingly large rejuvenation toolbox.

Sculptra won't be the right treatment for everyone — and that is exactly why consultation matters. Sometimes the best approach may be Sculptra. Sometimes it may be laser, skincare, microneedling, dermal filler, neurotoxin or a combination of treatments. And sometimes the answer is simply that we don't need to do anything right now.

The goal is never to fit you into a treatment. The goal is to find the treatment — if any — that fits you. Because rejuvenation should still look like you. Just you, supported.

Frequently Asked Questions

What is a biostimulator?

A biostimulator is an injectable treatment that encourages the body's own collagen production over time, rather than simply adding volume. Sculptra is a well-known example made with poly-L-lactic acid (PLLA).

How is Sculptra different from dermal filler?

Traditional hyaluronic acid fillers add or replace volume and can provide an immediate visible change. Sculptra works more gradually by stimulating collagen to improve the skin's underlying structural support.

How long does Sculptra take to work?

Results develop gradually as collagen production increases. Many clients notice changes over several weeks to months, with continued improvement across a series of treatments.

How many Sculptra treatments will I need?

A typical course is approximately three sessions over a few months, but the right number and timing depend on your individual goals and assessment.
